Principles of Evaluation
History
-
Premonitory Symptoms
-
-
Diplopia, vertigo, focal weakness (concern for neurologic cause)
Chest pain (concern for AMI, dissection, PE, aortic stenosis)
-
Shortness of breath (concern for PE, CHF)
Abdominal or back pain (concern for leaking AAA, ruptured ectopic)
-
-
Special Trauma Context
-
High suspicion if age >65 years, CAD history, or abnormal ECG
-
-
Diagnostic Testing in ED
ECG and ED Monitoring
-
-
-
Concerning findings
-
-
-
-
Prolonged QTc interval (>470 ms, with >500 ms associated with significant outcomes)
-
Laboratory Testing
-
-
-
-
Acid-base status: transitory wide anion gap acidosis follows generalized seizure but absent in syncope
-
-
Ancillary Testing
Carotid Sinus Massage
-
-
Requires continuous ECG and BP monitoring, informed consent
-
-
Contraindications: known carotid stenosis, bruits, stroke/MI within 3 months, history of VT/VF
-
-
Neurologic Testing
CT, EEG, LP have very low yield without trauma or focal deficits
