Distal and medial part of the 1/3 of the leg above the medial malleolus
Skin changes to look for
Hyperpigmentation
Venous stasis eczema
Pruritic, weeping scaling with erosion and crusting
Atrophie blanche
White ivory areas with hyperpigmented borders and telangiectasia
Lipodermatosclerosis
Firm, indurated, and hyperpigmented skin
May present with an inverse "champagne bottle" appearance
Chronic inflammation of subcutaneous tissue in the lower limbs
Ulcers
Arterial
Most painful
Over pressure areas and distal toes
Venous
Over medial malleolus
Edges are sloping down and brown / purple
Neuropathic
Painless
Most likely found over the heel and metatarsal heads
Buerger's Test
Positive: critical limb ischaemia
Lift patient's legs up to a 30-45 degree angle and hold for 30-60 seconds
If there is a change in color (pallor): positive
Rubor of dependency
Positive if toes turn red after hanging legs off the table afterwards
Venous dilatation

Less than 1mm
Varicose veins

Greater than 3mm
