Anatomical Relations of the Esophagus
Cervical Segment
Posterior Relations
Retrovisceral space
Vertebral column & prevertebral muscles (C6–C7)
Anterior Relations
Posterior surface of the trachea
Lateral Relations
Inside Visceral Sheath
Lateral lobes of thyroid gland & Parathyroid glands
Recurrent Laryngeal Nerves
Left: Lies in the tracheoesophageal groove
Right: Runs along the right border of the trachea
Outside Visceral Sheath
Sternocleidomastoid muscle (SCM)
Carotid Sheath (Neurovascular Bundle) :
Common carotid artery
Internal jugular vein
Jugulocarotid lymph nodes
Vagus nerve (CN X)
Thoracic Segment
Anterior Surface
Posterior surface of trachea
Left main bronchus
Dorsal surface of pericardium (separates esophagus from left atrium)
Posterior Surface
Thoracic duct
Azygos vein & tributaries
Thoracic vertebrae
Lower descending aorta
Left-Sided Relations: (Dense vascular/neural relations)
Left subclavian artery & Arch of the aorta (crosses left pulmonary hilum)
Left vagus nerve (gives off left recurrent laryngeal nerve)
Thoracic duct
Descending aorta (posterolateral surface)
Right-Sided Relations: (Relatively free/unobstructed)
Arch of the azygos vein (crosses right pulmonary hilum)
Mediastinal pleura
Right lateral tracheal lymph nodes
Vagal Nerve Migration (Esophageal Plexus)
Left Vagus (CN X) → Moves to left border → Anterior Vagal Trunk
Right Vagus (CN X) → Moves to right border → Posterior Vagal Trunk
Diaphragmantic Segment
Esophageal Hiatus
Muscular, extensible orifice at T10
Surrounded by a connective tissue sheath enabling gliding motion
Accompanying Structures
Anterior and posterior vagal trunks
Branches of cardio-esophagogastric & phrenic vessels (arteries & veins)
Abdominal Segment
Lateral Relations
Left
Fundus of stomach (separated by cardiac notch / angle of His)
Right
Lesser omentum
Anterior Surface
Posterior surface of left lobe of liver (leaves esophageal impression)
Left vagus nerve (anterior vagal trunk)
(Contained within the visceral sheath of the neck alongside the trachea)
(Traverses superior → posterior mediastinum; curves anteriorly and left toward T10)
