🚨 Low serum iron → NEVER rules out toxicity

💊 بعضی ترکیبات مثل Ferrous sulfate → دیده میشوند
❌ دیده نشدن → ردکننده نیست
🧲 DEFEROXAMINE ⭐
⚙️ Iron + Deferoxamine
→ Chelate
→ دفع کمپلکس از ادرار
🚨 Indications
☠️ Systemic toxicity
🤮 Persistent vomiting
🧪 Metabolic acidosis
📈 Progressive symptoms
🧲 Serum iron → moderate/severe toxicity
💉 Route
⭐ IV → روش توصیهشده
💉 IM → در بیمار بدون Shock ممکن است
💊 Dose
👨 Adult initial → 1000 mg
👶 Child initial → 50 mg/kg
➡️ Infusion → 5 mg/kg/h
💧 IV fluid → برای جلوگیری از Hypotension
⚠️ Hypotension
❌ Contraindication مطلق نیست
🚨 High-dose Deferoxamine
→ Mucormycosis
→ Renal failure
→ Pulmonary toxicity
→ Yersinia sepsis
🟠 Vin Rose / Red urine
🧲 Iron-Deferoxamine complex → ادرار تغییر رنگ میدهد
🛑 Stop Deferoxamine
⭐ مهمترین معیار → Clinical improvement
🧲 Serum iron → Normal
🟠 Urine → Normal color
🩸 Coagulopathy
→ Vitamin K
→ در صورت نیاز FFP
🩸 Severe bleeding → Blood transfusion
🧪 GI decontamination
❌ Activated charcoal → توصیه نمیشود
❌ Cathartic → توصیه نمیشود
❌ Oral sodium bicarbonate → توصیه نمیشود
⏱️ Lavage → 60 min اول یا SR preparations
🩻 Iron در X-ray → احتمال Hepatotoxicity
👶 Children + large ingestion
→ PEG مفید

→ خارج کردن مقدار زیاد
→ Bezoar

→ حدود 20–35 قرص acute ingestion
