Any triggers or changes at that time? (ie; moving, relationship, job change)

Context/triggers, cognitions, emotions, behaviours and physical symptoms
detailed account - where? whats happening? how often? AND whats going through your mind? whats happening in their body? etc

Maintaining factors

Example: anxiety. look for avoidance behaviours, what consequences are associated with avoidance?

Example: depression - dont socialise, dont engage in activities, friends may retract, client becomes isolated, reinforces depression
