Sepsis
Definition
Clinical Syndrome
Dysregulated host response t o infection
Rapidly progressive
Multiorgan dysfunction and death
-
Sepsis
(1) Infection + (2) dysregulated host repsonse to infection + life-threatening (3) organ dysfunction
-
Epidemiology
-
Increasing rates
- Increasing age
- Immunosuppresion
- Drug resistance
- Awareness
- Increased detection
RF
- ICU - 50%
- Male
- Increasing age
- Immunosuppression
- DM and obesity
- CAP
- Seasonally winter
Pathophysiology
-
- Infection trigger
- Immune activation
- Systemic inflammation
- Endothelial dysfunction
- Organ dysfunction
-
Prognosis
-
-
Factors
- Hypothermia
- Low WCC
- Age
- Hyperglucaemia on admission
- UTI lowest mortality
- Ischaemic bowel highest morta
- Nosocomia - high
- Early antimicrobials - 50% reduction in mort
-
Organ Dysfunction
-
Assessment
- SOFA score ( Sequential Organ Failure Assessment Score )
-
-
-
Clinical Presentation
- Hx infection
- Hypotension
- Tachycardia
- Fever of >38.3 or <36
- Tachypnoea
- signs of altered end organ perfusion

Sepsis 6
1 hour
- Inform senour
- Maintain patent airway
- Give O2 to maintain >94%
- IV acess, fluids 30ml/kg crystalloid, inotrope if still hypotensive - Noradrenaline
