-abdominal pain
-back pain
risk factors
-hypertension
-cocaine use
-underlying aortopathy (marfan syndrome)
diagonisis:
-enlarged aorta (chest x ray)
-flase lumen( ct scan of the aorta)
-aortic lumen walls (transesophageal echocardiogram)
complications
-at risk for life-threatening aortic rupture-cardiac tamponade
-extension of the dssection may cut off blood supply to critical areas
intervetion
target reduce heart rate and blood pressure
-beta adrenergic antagonist (iv labetalol or semolol) heart rate first then blood pressure
-keep the client in bed in semi-fowler postion
-maintain a quite environment
-keep blood pressure between 100-120
may include insertion of an arterial catheter
-continusous cardiac monitoring
- iv morephine
-montior for complications like (stroke, cardia tamponade heart failure)
-prepare the clinet for emergnecy surgery for type a
