Gradual onset
HTN x 12 years
Pathophysiology
Impaired CSF absorption at arachnoid granulations
CSF gradually accumulates, ventricles compensate
ventricles gradually enlarge
periventricular motor tracts compressed
difficulty picking up feet
wide-based gait
shuffling with ambulation
"feet won't do what I want"
Falls (2)
lower extremity weakness
compression of frontal subcortical circuits
forgetfulness
agitation
behavior changes
slowed thinking
flat affect
appears older
slowed movements
stretching of corticospinal tracts
slow lower extremity DTRs
mild imbalance
RAM with poor coordination
stretching of basal ganglia circuits
mild, resting left hand tremor
widespread brain dysfunction and subsequent functional decline
fatigue
weight loss
decreased appetite
