Perineal surgery
Correction of vaginal/perineal tears
mucosal only tears don't need repairing
grade 2 + 3 tears will benefit from surgical repair
post calving "check for tears and spares"!!
epidural anaesthesia
use cotton wool filled rectal clove to prevent faecal contamination
clean
repair rectal mucosa, then intervening layer, then vaginal mucosa, then perineal skin
Episiotomy
= incision into the perineum
indications
inadequate dilation--> assisted delivery
conformational abnormalities
scarring from previous dystocia
procedure
incision at 2 and 10 o'clock
dorsolateral direction avoids rectovestibular laceration
close immediately after calf delivery
Ovariohysterectomy (not D1)
preventative surgery/therapeutic surgery in pet pigs
referral
Correction of old 3rd degree perineal tears
if contaminated, or much swelling, leave to heal by secondary intention and then repair surgically
if not contaminated
if shallow, remove a triangle of skin/mucosa and submucosa on each side and appose
if deeper, split the rectovestibular shelf in the frontal plane and appose the free edges
