It is an assessment of the education, healthcare and social care needs of the child or young person
The EHCP is a legal document which sets out a description of a child's needs. It documents what they can and cannot do and what is done to meet those needs.
Fair Access to Care
This guidance provides councils with social services responsibilities with a framework for determining eligibility for adult social care.
It covers how councils should carry out assessments and reviews, and support individuals with through these process.
Councils should ensure that they provide or commission services to meet eligibility needs.
Councils should assess an individuals presenting needs, and prioritize their eligible needs, according to the risks of their independence in both long and short term where help may not be provided

Individual Budget
This is the money that individuals use to buy the support and services they need to meet the outcomes they have identified in their support plans.
Housing Adaptations
Adaptations may include stair lifts to help people access upstairs rooms, widening doorways and building ramps so people can get around their houses.
Meeting peoples housing needs
If an individual is not able to stay in their homes due to their needs then they may be entitled to adapted accommodation. For example, a ramp to access their house
Sheltered housing
houses that have onsite carers to help you live as independently as possible.
Residential home for people who cannot live unsupported.
Choice of Residence
For local authorities, personalization means offering individuals choice over where and how they live
Local authorities must ensue that the house is accessible and designed with individuals needs in mind
Meeting housing needs
If an individual want to stay in their home, social services should do their best to help them stay there with the help of specialized equipment and adaptations
If it is not possible to adapt the house then the options would be;
Purpose built or adapted accommodation that is accessible and meets the individuals needs; for example, a ramp might already be in place to the front door, or it may have wider doors to accommodate a wheelchair.
Sheltered accommodation that gives an individual independence of having their own bungalow or flat.
A residential home that would be suitable for an individual who finds it difficult to look after themselves and cannot receive the care needed in their own home.
The Care Act
Removal of geographical barriers
This means that regardless of a person moving to a new areas, their acre will not change regardless of geographical barriers
LA’s should know when a person is moving areas and put provisions in place to ensure that the person's needs are still met in the new area.
Decentralization and commissioning
Outsourcing services
This means that some services are provided by a third party and not the local authority however it is equal to the NHS.
Promoting greater range of choice
This means that service users can choose from a range of different providers not just the NHS.

Before personalization, the services on offer to an individual would come from inside the local authority. If the service was not available then the individual had no choice. Now services can be sourced from many different organisations, so the individual can choose the service that best meets their needs
