Pathophysiology: Bronchospasm + inflammation → narrowed airways, reversible obstruction
Assessment: Wheezing (expiratory), accessory muscle use, prolonged exhalation, “silent chest” (ominous)
Treatment: High-flow O₂, assist with MDI/nebulizer (albuterol = β₂ agonist), position of comfort, assist ventilations if inadequate

Assessment: Barrel chest, pursed-lip breathing, rhonchi/wheezes, chronic cough with sputum
Treatment: O₂ (do not withhold if hypoxic), bronchodilator (albuterol), transport

Assessment: Crackles/rales, pink frothy sputum, severe dyspnea, orthopnea, JVD
Treatment: High-flow O₂, upright position, CPAP if available, transport, nitro per protocol if hypertensive and no ED drugs

Assessment: Fever, productive cough, localized crackles, pleuritic chest pain, tachypnea
Treatment: O₂, supportive care, transport

Assessment: Sudden dyspnea, tachypnea, chest pain, hemoptysis, clear lungs, risk factors (immobilization, surgery, DVT)
Treatment: High-flow O₂, transport, notify ALS/hospital

Assessment: Sudden chest pain, dyspnea, diminished breath sounds one side, tachycardia
Treatment: O₂, position of comfort, transport

Pathophysiology: Air trapped under pressure → mediastinal shift, obstructive shock
Assessment: Severe dyspnea, absent breath sounds one side, hypotension, JVD, tracheal deviation (late)
Treatment: High-flow O₂, ALS needle decompression, rapid transport
Key notes: Life-threatening, rapid recognition critical

Assessment: Barking cough, stridor, fever, worse at night
Treatment: Humidified O₂, keep child calm, transport if distress

Assessment: Drooling, tripod position, high fever, stridor, muffled voice
Treatment: Do NOT put anything in mouth, keep child calm, O₂, rapid transport

Pathophysiology: Systemic allergic reaction → bronchospasm, airway swelling, vasodilation
Assessment: Urticaria, angioedema, wheezing, stridor, hypotension, tachycardia
Treatment: IM epinephrine (adult 0.3–0.5 mg, pedi 0.01 mg/kg max 0.3 mg), high-flow O₂, assist ventilations as needed, rapid transport
Key notes: Early epi is lifesaving, may need repeat doses
