LP - Priority Area 2 - Health Services Literacy & Awareness
Low use of healthdirect (11%)
Promotion of healthdirect through media and/or within care navigation solutions
Local service directories are incomplete; information isn’t consistently shared across providers
Can we improve/upgrade/update local service directories?
Why incomplete?
Ongoing time/cost committment
Difficulty getting info from services
Info keeps changing
What service directories already exist?
HealthDirect
My Community Directory mycommunitydirectory.com.au
Ask Izzy
2020 Liverpool Plains Community Directory
Who is responsible for current directories?
LPSC
Services themselves
Community Orgs - eg Sora
Currently good access to a range of allied health services
Josie – Scone Hospital showed up with a stall to a community event for face to face engagement – can we be at the Show, do a roadshow, go to AgQuip, markets
Good range available but still a priority area means lack of awareness
Referrals get tricky when one org does lots of different programs with different referral pathways, especially when it’s run by the LHD, people often think they can just call the hospital, but the hospital has no idea what they’re talking about.
Existing Care Navigation Services
We Care Disability Services
People often ask at the hospital/MSP (they aren't always equipped to help)
Benevolent Society
Sora
People often ask TAMS (they aren't always equipped to help)
Lack of services (have to travel to Tamworth/Newcastle/Sydney for specialists)
Telehealth great for follow up appointments / results appointments
Support needed to book multiple appointments in one day
HealthWISE receiving fewer referrals, patients even being told not to go to HealthWISE by PHN funded service
Nurse Practitioner Model
NPs provide patient education on conditions, medications, and self-management, empowering patients to take control of their health.
Stigma around Nurse Practitioners (mentioned in health professional interview)
Pharmacy Model
Pharmacists can educate on medication use, side effects, and adherence, especially for chronic disease patients.
Health education Flyers/Posters
Posters from Snowy Valleys project
showing what different doctors and allied health people do in plain English
Display at commonly attended locations eg library, shopping centre, mens shed, CWA, sporting grounds, etc
Buy a digital billboard and display health information?
