Mini Mental State Examination (MMSE)
Purpose
To screen for client's cognitive impairment.
To assess client's cognitive function.
To monitor disease progression or treatment effectiveness.

Explain the purpose of the assessment to the client in simple terms.
Instruct the client to complete the task that was given verbally.
Be neutral and avoid giving hints
If the patient has difficulty due to physical impairments, note that separately.
Scoring
18 - 23 (Mild cognitive impairment)
0 - 17 (Severe impairment)
24 - 30 (Normal cognition)
