FUNCTIONAL ABILITY ASSESSMENT
Purpose
To determine client's level of independence
To evaluate client's return-to-work capacity
To guide treatment and rehabilitation planning

Assessment's Administration
Gather the patient's personal family history, work history, medical history, and side effects of medication.
Therapist interview the client using the functional ability assessment form.
Explain the purpose, process, and rights of the client
Therapist assess the client using observation and physical assessment.
Use standardized scoring where applicable.
Review results with the client and discuss goals, limitations, and next steps in rehabilitation.
Scoring
1-5 rating scale for each categories
92 - 117 - Need constant assist/prompting
118 - 130 - Dependent
66 - 91 - Need intermittent assist/prompting
27 - 65 - Need min. assist/prompting
26 - Independent
Each scores based on its specified description.
Can be rated as "E" if client needs further evaluation, or "NA" if not applicable.
