THRESHOLD ASSESSMENT GRID (TAG)
Purpose
Determine the level of need and urgency of a client requiring support
Determine the severity of the client current situation.

Survival
Risk from others
Psychological
Unintentional self harm
Social
Intentional self harm
Assessment's Administration
Conducted by interviews between therapist and the client.
The therapist will tick one statement for 7 different domains according to the client's response.
Therapist can decide if the client meets the threshold for particular service.
Scoring
Statements represents different scores for each domain.
Moderate (2 pts.)
Severe (3 pts.)
Mild (1 pts.)
Very severe (4 pts.)
None (0 pts.)
Severity level and Response Time
2 or > severe ratings = Refer between 72 hours
2 moderate ratings = Refer between 2 weeks
1 very severe ratings = Refer between 24 hours
< 2 moderate ratings = Depends on specialist judgement based on the referral letter
