Assessment's Administrations
This assessment can be completed either by self-completion or by interview.
Therapist/client will tick one of the scores based on the client's responses.
Therapist can determine which categories in client's daily life occupation that requires assistance.

There are three scores for each answers
Met need
Unmet Need
No Need
If it's personally disturbing, the client have the right to avoid the question by ticking the "I don't want to answer" on the last row.
