Pathogenesis
1.endocardial surface is injured by shear forces associated with turbulent blood flow (from CHD or CVL)
- at the site of endothelial damage, platelets, fibrin and sometimes red blood cells will get deposited forming a noninfected thrombus.
- Transient bacteremia or fungemia occurs when microbial pathogens adhere to the injured endothelium and thrombus. additional fibrin and platelet deposition over the infected vegetation resulting in a protective sheath.

anemia, elevated inflammatory markers, urinalysis showing hematuria.

Neonates

feeding intolerance, tachycardia, respiratory distress, hypotension, new or changing murmur.

right sides IE from CVL may have limited clinical evidence of diasease besides persistent positive blood cultures with the use of appropriate antibiotic therapy.
