Characteristics: What is the colour consistency and order of the discharge? I sit clear white, yellow, green or grey? Is it thin thick frothy or clumpy? Does it have a fishy, yeasty,,or foul odour
Associated Symptoms: Are there any symptoms like itching, burning, pain, or swelling?
Menstrual History: Is the discharge related to the menstrual cycle? For eg: changes before, during, and after menstruation.
Other Factors: Dispareunia, Abdominal pain, abnormal vaginal bleading, and fever.

Medication usage like antibiotics, streoids, etc.

Examination
Bi-Manual Pelvic Examination for Adnexal and Uterine tenderness, mass, and cervical motion tenderness
Vulvar Inspection for discharge, erythema, ulcer, and other lesions including skin changes

Speculum Examination to vaginal walls, cervix, and characteristics of discharge. Endocervical swab if there is risk of STDs.
