Coordination, Cranial N. Testing - Coggle Diagram
Coordination
Balance
definition
Ability to orient to the surrounding environment while maintaining the COG within the BOS (very complex)
-
-
Balance tests (2 types)
static
seated balance
-
things to monitor (4)
posture, compensation on one side than the other, seat height (so foot can touch the ground), if they can reach
two ways to perform
side-to-side or front-to-back weight
shifts, to reach in various directions
-
standing balance
3 types
-
-
Single leg stance
rules of the test (3)
Eyes open (EO) or eyes closed (EC)
• Firm surface, no shoes, arms at sides
• Standing on one foot with other raised
-
-
-
dynamic
standing balance
reactive test
-
what is the test called
Nudge/push test
what occurs
giving pt a nudge at random intervals from the front, sides, and back; Start light to increase force as pt able to tolerate

-
-
seated balance
what can be performed
side-to-side or front-to-back weight
shifts, to reach in various directions with one UE or both UEs moving
-
In any given situation, when an individual experiences a loss of balance, one of two things will happen
invoke a strategy to correct the problem, or
the individual will fall
-
what are the 2 standardized questionnaires that have proven
clinically useful to know if a pt is confident w balance
-
-
requires what 3 things
Requires adequate ROM, strength, sensory pathways, cognition
-
Cranial N. Testing
-
Upper Motor Neuron test
Muscle tone
Hypotonia
-
how would it feel
when limb is passively moved, there is a "floppy" feeling to the movement and little to no resistance
-
Hypertonia
-
how would it feel
when passively moved increased resistance is felt, especially when the muscle is placed in a stretch position
-
Spasticity
description
manifestation of hypertonicity in which
the resistance to passive motion is rate or velocity dependent
-
-
-
How to test
-
PT passively moves the pt in a planar motion to check if you feel resistance (a pull; hypertonia) or no resistance at all (hypotonia)
where can you test
ankle, knee, hip, wrist, elbow
when going fast in hip flexion, what m. tone are you testing
hip extensors, so its always the opposite
why is this an UMN test
UMNs in the brain directly influence and regulate the level of muscle tone by controlling the activity of lower motor neurons in the spinal cord
-
Clonus Test
definition
rapid, involuntary back and forth beating, continues multiple cycles
-
test (4 things)
pt relaxed/supported, knee slight flexed, slow ankle PROM, then quickly to end range dorsiflexion
-
-
Pronator Drift
how to test
Flex shoulder to ~90˚ , forearm supinated
with eyes closed
-
-
Babinski
how to test
position
Pt supported supine, on skin of foot use capped pen or reflex hammer end
-
results
-
abnormal
great toe extension and other toes splay apart (“Positive Babinski“), flexor withdrawl reflex
Hoffman's test
how to test
Hold pt’s 3rd distal phalanx between finger and thumb
Quickly slide thumb off pt’s nail (into DIP flex)
