Definition
-
Indicated for infection of abdomen (abscess/diverticulitis), injury to colon/rectal, intestinal obstruction, rectal/colon cancer, perineum fistula
Surgical procedure that brings one end of the large intestine out through an opening (stoma) made in the abdominal wall

Stoma
Rounded, red and moist stoma (pallor may suggest anaemia; dark hue may represent ischaemia)

No evidence of erythema, rash, ulceration or inflammation in the peristomal skin

To empty the colon of gas, mucus, feces

Measure stoma size
On the backing of the skin
barrier, trace the measured pattern on pouch

Remove pouch
Remove the pouch slowly, begin at the top and work downward while holding the patient’s skin taut

Prepare patient

Place bath blanket over client, protective sheet under client

Emptying pouch

While wearing gloves, hold pouch outlet over a bedpan/ toilet
Dispose used items, remove and discard gloves, wash hands

Record discoloration of stoma, appearance of peristomal skin, amount and type of drainage, reaction to procedure, experience with ostomy and skills learned by patient
