Neurological Disorders &Disorders of Voice Use
Neurological Disorders Affecting Voice
Peripheral Nervous System Pathology
Superior Laryngeal Nerve Paralysis
Unilateral or Bilateral
Symptoms: Loss of pitch range, vocal fatigue, breathiness
Etiology: Surgical trauma, cardiovascular disease, thyroid surgery
Diagnosis: Videostroboscopic signs, epiglottic petiole deviation
Treatment: Observation, voice therapy
Recurrent Laryngeal Nerve Paralysis
Unilateral
Symptoms: Dysphonia, breathiness, low pitch
Laryngeal Signs: Incomplete vocal fold closure, weak fold tone
Bilateral
Symptoms: Severe respiratory difficulty, airway protection issues
Treatment: Tracheostomy or arytenoid manipulation
Myasthenia Gravis
Symptoms: Vocal weakness, breathiness, reduced pitch
Diagnosis: Electromyography (LEMG)
Treatment: Acetylcholinesterase inhibitors, voice therapy
Movement Disorders Affecting the Larynx
Spasmodic Dysphonia
Types: Adductor, Abductor, Mixed
Symptoms: Strained voice, voice breaks, breathy phonation
Treatment: Botox injections, behavioral voice therapy
Essential Vocal Tremor
Symptoms: Regular pitch and intensity wavering, voice breaks
Diagnosis: Laryngoscopic signs
Treatment: Medication, voice therapy
Central Neurologic Disorders Affecting Voice
Parkinson’s Disease
Symptoms: Monopitch, monoloudness, breathy voice, facial rigidity
Treatment: Levodopa, deep brain stimulation, Lee Silverman Voice Therapy
Multiple Sclerosis
Symptoms: Spastic-ataxic dysarthria, intermittent aphonia, impaired coordination
Treatment: Functional communication preservation
Amyotrophic Lateral Sclerosis (ALS)
Symptoms: Spastic-flaccid dysarthria, slow speech, breathy voice
Treatment: Compensatory strategies, augmentative communication
Huntington’s Chorea
Symptoms: Strained voice, involuntary phonatory bursts
Treatment: Speech therapy, augmentative communication if needed
定義
由於支配喉部的神經傳導中斷(包括中樞神經系統和周邊神經系統)而直接引起的語音障礙。"
Disorders of Voice Use
Vocal Abuse, Misuse, and Phonotrauma
Symptoms: Hoarseness, vocal fatigue, limited pitch
Risk Factors: Loud environments, inadequate vocal technique
Management: Vocal rest, behavioral therapy
Muscle Tension Dysphonia
Types: Primary (no pathology), Secondary (compensatory behavior)
Symptoms: Neck pain, tension in the laryngeal area
Treatment: Laryngeal relaxation techniques
Ventricular Phonation
Description: Use of false vocal folds for phonation
Symptoms: Rough, low-pitched voice, limited endurance
Treatment: Behavioral therapy to encourage true fold use
Paradoxical Vocal Fold Motion (PVFM)
Symptoms: Stridor during inhalation, respiratory difficulty
Triggers: Exercise, psychological stress, environmental irritants
Treatment: Breathing techniques, visual biofeedback, antireflux medication
