CHRONIC WOUNDS
(can't pass through all phases of wound healing- gets stuck into a particular phase)
Can take >6 weeks and can persist >3 months
Destruction of all layers of skin
Result usually from complications
Foot ulcers from diabetes, pressure ulcers resulting from spinal cord injuries
Poor nutrition or wound infections.
ACUTE WOUNDS
Take around 3-6 weeks to heal as it involves superficial layer
of skin
Mainly surgical incisions, thermal wounds, abrasions,
and lacerations etc.
Progress through all the phases of wound healing
STAGES OF WOUND HEALING :face_with_head_bandage:
HEMOSTASIS🩸
INFLAMMATION :explode:
PROLIFERATION :!!:
MATURATION / REMODELING :hammer_and_wrench:
Collagen is remodeled and wound fully closes
Wound is rebuilt. Fibroblasts produce collagen. Epithelial cells reformed.
Localized swelling controls bleeding, healing cells arrive, WBCs clear our debris and prevent infection
Vessels constrict + Clotting
DEBRIDEMENT🧹
Involves thoroughly cleaning the wound and removing all infected, and nonviable (necrotic
or dead) tissue and foreign debris
AUTOLYTIC🩹
The most used method of debridement that uses the body's own enzymes and moisture to rehydrate tissue
Examples: hydrogels, hydrocolloids, and hydro fibers
ENZYMATIC :green_cross:
Debridement of using an exogenous proteolytic enzyme
:+1: Easy, Highly Selective
:-1: Contraindicated for infected wounds, Topical agents can inactive enzymes
MECHANICAL :hammer_and_wrench:
Will remove both devitalized and viable tissue. Carried out using mechanical force: wet-to-dry, pulsatile lavage, or wound irrigation
Examples: lavage
:+1:Easy, Faster than autolytic and enzymatic
:-1: Can be painful, May damage surrounding tissue
BIOLOGICAL :bug:
Living organisms to remove necrotic or dead tissue
Example: maggots
:+1: Highly selective, fast, painless
:-1: Patient discomfort, only for selected cases
DRY vs WET WOUNDS
DRY :hourglass:
Increased incidence of infection
Possibly increased pain
Up to 2x longer to heal as a moist wound
WET :sweat_drops:
Chronic wound fluid is pro-inflammatory
Can cause maceration to the peri-wound causing a larger wound
Too much drainage can result in more frequent dressing changes, pain, resources, and money
