Occupation and Daily Life
: Work hours, lifestyle, activity levels, and work-related stress.
Sleep Hygiene
: Sleep patterns, sleep environment, use of electronics before bed.
Family Life
: Family stress, care for dependents (e.g., elderly or children).
Associated Symptoms
General Symptoms
: Weight changes, fever, night sweats, muscle aches, or pain.
Sleep-related Symptoms
: Difficulty falling or staying asleep, unrefreshing sleep, snoring, sleep apnea.
Mental Health
: Stress, depression, anxiety, emotional instability, or cognitive difficulties.
Cardiovascular or Respiratory Symptoms
: Shortness of breath, chest pain, palpitations.
Endocrine Symptoms
: Cold or heat intolerance, polyuria, polydipsia, weight gain or loss, changes in appetite.
Gastrointestinal Symptoms
: Nausea, vomiting, abdominal pain, or diarrhea.
Infectious Symptoms
: Recent infections, sore throat, swollen glands.
Psychosocial and Emotional Factors
Major life changes (e.g., death of a loved one, relationship issues, job loss).

Severity
How has the fatigue impacted daily activities (work, exercise, social interactions)?
Has the patient had to reduce activities due to tiredness?
Character of Fatigue
Is the fatigue constant or intermittent?
Is it more pronounced at certain times of the day (morning or evening)?
Does rest improve it, or is it unrelieved by sleep?
Travel History
Recent travel, especially to areas with endemic infections (e.g., malaria, Lyme disease).
Exposure to unusual environments or high altitudes.
Onset and Duration
When did the fatigue start?
Is it sudden or gradual?
How long has it persisted (days, weeks, months)?
Family History
Family history of chronic diseases, particularly those that can cause fatigue (e.g., thyroid disorders, autoimmune diseases, cancers, mental health issues).
Medical History
Chronic Conditions
: Diabetes, thyroid disorders (hypothyroidism, hyperthyroidism), chronic kidney disease, heart disease, lung disease, anemia, autoimmune disorders, or cancer.
Medications
: Current medications, recent changes, side effects (e.g., sedatives, antidepressants, antihypertensives).
Substance Use
: Alcohol, drugs, caffeine, tobacco.
Allergies
: Any recent allergic reactions or intolerance to foods or medications.
