All elements are essential for the full assessment of a patient; GCS, pupillary response, motor/sensory assessment and vital signs.

Painful Stimuli - This assess the power and movement of limbs . Two types of stimulus: central and peripheral
Central stimulus should be used to assess best upper limb motor response if the patient cannot obey commands, which is also recommended by the National Neuroscience Benchmarking Group (Waterhouse 2009).
Central stimulus allows the assessment of the higher centres of the brain where reflex action response cannot be activated (Waterhouse 2009).
Peripheral stimulus is deciphered by the peripheral nervous system which is then communicated to the CNS, however a reflex response can be activated (Palmer and Knight 2006).

Supraorbital notch pressure - pressure applied to superaorbtial notch - contraindictared in orbital damage, skull fracture and glaucoma.

Pupillary responses -
Each pupil must be assessed for shape, size and reaction to light.

Reaction – Pupils should react to light quickly (+), abnormal reactions include: slower to react (sluggish (sl)) or non reactive (-)
Any pupils that are unequal in size/ shape/ has an abnormal reactivity must be reported immediately.
