Begins when infection irritate cough receptors in airways not alveoli

BTS vs NICE Guideline
- Both recommend amoxicillin 1st line
- IV reserved for those unable to tolerate fluids, with sepsis or complicated pneumonia
- Azithromycin not rec as 1st line macrolide due to concern that long half life may lead to abx resistance

IF a child remains feverish or unwell 48hr after tx , re-evaluation should be considered given to possible complications

Clinical Features
-
Coryzal prodrome 1-3 days followed by:
- Persistent cough and
- Tachypynoea and or chest recession and
- wheeze and or crackles

Assessment
-
- Suspect impending respiratory failure
Exhaustion
Listlessness
Decreased respiratory effort
Recurrent apnoea
Failure to maintain O2 despite supplements

- Do not routinely perform blood tests or CXR
- Consider CXR if ICU is proposed
- Provide parents / carers with key safety information if child is not admitte

Admission
- Persistent O2 saturation of <90% in infants 6/52 and over
or <92% infants < 6/52 or children with underlying health conditions
- Inadequate Oral fluid intake (50-70% of usual volume)

- Apnoea or persisting severe respiratory distress
Grunting
Marked chest recession
RR >70 breath/min
Management
-
- Respiratory distress or feding difficulties due to secretions
- Apnoea even if there are no obvious secretions
- Respiratory support if required
- High flow O2
- CPAP
- NIV
- Mechanical ventilation

- Infection control measures - RSV is highly infectious
