Urinary/Renal system
The Kidneys
Overview
Reddish-brown, bean-shaped, paired retroperitoneal organs on either side of the vertebral column T12-L3
-
Dimensions: 10-12 cm in lenght, 4-6 cm in width, and 3-4 cm in anteroposterior dimension
-
Anterior relation
right kidney
liver(right lobe), right colic(hepatic) flexure, duodenum (descending part)
left kidney
stomach, spleen, pancreas, left colic (splenic) flexure, jejunum
-
-
-
-
-
Nephrons
proximal convoluted tubule are formed by cuboidal epithelial cells with large mitochondria and dense microvilli
-

-
Urinary bladder
Overview
Located extraperitoneally, behind the pubic symphysis and beneath the peritoneum
Can hold 500-1000 ml, but sensation of bladder fullness at 300-500 ml
Hollow, triangular-shaped organ
Microscopic anatomy
Epithelium: transitional epithelium, empty bladder (5-6 layers), full bladder (3-4 layers )
-
Structure
-
Neck
-
Characteristics: 1- comprise the internal urethral sphincter, 2- attached to the posterior surface of the pubic via the pubourethral ligament
Body
-
Characteristics: 1-supported inferiorly by prostate and pelvic diaphragm in males and females respectively, 2- supported laterally by the lavator ani and internal obturator muscles
Trigone
-
Formed by the two ureteral openings superiorly (base), and the openings of the urethra (apex)
Apex
-
Characteristics: 1- attached to the median umbilical ligament, 2- superior portion is covered by peritoneum
Vasculature
Blood supply
-
Branches of the obturator artery & vein, and inferior gluteal artery & vein
-
-
Innervation
Sympathatic: Hypogastric nerve (T12-L2). It cause relaxation of the detrusor muscle (B3 receptors) promoting urine retention
Parasympathetic: pelvic nerve (S2-S4). It cause contraction of the detrusor muscle, (M3 receptors) stimulating micturition
somatic: pudendal nerve (S2-S4). It innervate the external urethral sphincter, providing voluntary control over micturition
-
Ureters
Overview
The ureters are paired, retroperitoneal tubes which act to transport urine from the kidneys to the bladder
They are approximately 25 cm long suited bilaterally,

Urethra
Male urethra
-
-
-
-
Epithelium
-
Spongy urethra
pseudostratified columnar epithelium, stratified columnar epithelium in terminal portion
-
-
These vessels originate from two lymphatic plexuses- one deep to the capsule, and the other in the medulla
