Diagnose
-
Associated symptoms - headache, nausea, vomiting and abdominal pain (children with pharyngitis) and all people with tonsillitis.

Fluid intake if pain on swallowing/ number of wet nappies for babies, reduced skin turgor or capillary refill > 3 sec /dry mucous membrane
Use FEVERPAIN criteria or CENTOR clinical criteria score - to determine strep infection and need for antibiotics
Other signs / symptoms Common cold - rhinorrhoea, nasal congestion and cough.
Tonsillar exudate, absence of cough
Management
-
Hospital admission for e.g. breathing difficulty/clinical dehydration/peri-tonsillar abscess / parapharyngeal abscess, retropharyngeal abscess or Lemierre syndrome, marked systemc illness or sepsis or suspected rare rash e.g. diphtheria

DMARD - if admission not appropriate, take FBC, Withhold DMARD until results and discussion with Rheumatology specialist, low WCC seek urgent specialist advise /refer. Consider antibiotics.
Carbimazole - if person taking, take FBC urgently and w/h drug until result available, seek specialist advice, consider antibiotic.
Chemotherapy - with suspected leukaemia/HIV/AIDS , seek immediate specialist advice /referral, check FBC

If fever score 2 or 3, no antibiotic required or back up antibiotics if symptoms not improved 3-5 days.
Treatment

Clarithromycin if penicillin allergy, erythromycin for pregnant woman with penicillin allergy
