• Recognise the different stages of an Incident, Hazard and Feedback Investigation

• Complete an Incident submission

• Complete a Feedback submission

• Understand the process of investigation and Closing the Loop

This module is provided to all staff with a ‘Responsible Managers’ delegation.

• Provide opportunities to consolidate Datix: End User module learnings

• Participants will demonstrate the ability to navigate the landing page of Datix – both Incident and Management (I&M) tab, and Consumer Feedback (CF) tab.

• Participants to demonstrate the ability to understand the submission process for I&M and CF Datix, complete some basic tasks for the investigation, and close of a Datix.

• Participants to demonstrate the ability to Investigate and close a Consumer Feedback Datix.

• Be able to navigate 3 Case-Studies. 1. Client I&M. 2. Staff I&M and 3. Suggestion CF.

NEW -two and a half hour workshop consisting of 3 parts. Facilitator present throughout initially to guide direct and answer questions while participants to the eLearning, and then to facilitate the participants practicing. ✅

Incident Reporting and Open Disclosure (Asynchronous eLearning) This one is mandatory for clinicians

This course will provide you with an understanding of the incident reporting process, including the principles of open disclosure, and how these are applied in the Silverchain group.

• identify the difference between an incident, near miss and hazard

• use the incident reporting process with confidence

• demonstrate the benefits of the reporting and open disclosure processes

• apply the knowledge and skills gained to actively participate in the incident management process

• understand serious incident response scheme (SIRS) mandatory reporting.

Face to face - Hands on scenario-based practice in the Datix training environment – 60mins The training approach is shifted from focusing solely on the mechanics of using Datix to understanding WHY it serves as a valuable clinical tool, followed by practical guidance on its usage.

During this phase, participants are encouraged to create a Datix entry using the provided resources. The facilitator’s role is to prompt and guide participants through the Datix entry process, emphasising practical application rather than demonstrating an end-to-end procedure.

Integrate real-world clinical scenarios into the training. Use case studies, patient stories, or simulated situations to illustrate concepts. These scenarios need not be overly complex; the goal is not to quiz participants on immediate incident responses but to deepen their understanding of Silverchain’s client incident management procedure using Datix.
