Growth plates & Orthopaedic considerations
Need to be aware of growth plate closures when viewing XR (normal growth plate vs fracture)
Subjective Ax
Pain behaviour - AM/PM, VAS
P/N/N, sensorineural changes
Type of pain - sharp, dull
Pain at rest or activity, decr or incr w warmup
Duration, constant or intermittent
Concomitant symptoms + red flags - fevers, headaches, night pain, urinary changes
Previous treatment
Type of activity or change to activity/ loading
Psychosocial - sleep, nutrition, social, school life, mood
Training load + cross training + rest days
Recent growth spurts
Objective Ax
Observation of area - redness, heat, oedema, bruising
Palpation
ROM & strength
Functional assessments
Balance & coordination
Common injuries
Oschgood Schlaters + PFPS
Stress fractures
Overtraining + REDs (Relative Energy Deficiency Syndrome
Psychosocial issues, Orthorexia, exercise addiction
Sever's lesion
Osteitis Pubis
Vertebral stress fractures - gymnasts, cricket
Social influences
Eager parents
Friends doing the same sport
Social media influences
Overtraining
REDs (Relative Energy Deficiency in Sport) - females
Low mood or motivation to train, disinterest
Childhood obesity + not enough exercise as a contrast
Imaging
Treatment + rehab
Will depend on injury and SubjAx findings
Education++++, to both patient and parents
Working with coaches, school teachers to provide advice where appropriate
