Anxiety Disorders
Panic disorder
Panic attack (specifier)
Discrete periods of intense anxiety associated with various physical symptoms (trembling, shaking, sweating, palpitations, SOB) and cognitive symptoms (fear of losing control or going crazy)
Recurrent, spontaneous & unexpected panic attacks (no specific causative factor)
Becomes disorder when:
- Persistent worry about another attack
- Worry about implications of attacks
- Significant change in behaviour because of attacks
Differential diagnosis
Medical
- CVS: anaemia, angina, HF, supraventricular tachy
- Pulmonary: asthma, hyperventilation, PE
- Neuro: TIA, CVA, epilepsy, Meniere's disease, migraine
- Endo: Addison's, carcinoid syndrome, hypoglycaemia, hyperthyroidism, phaeo
- Substances: amphetamines, stimulants, cannabis, hallucinogens, nicotine
- Withdrawal: alcohol, benzos, opiates
- Other: anaphylaxis, electrolyte distrubances
Psychiatric
- Common: MDD, SAD, specific phobias, PTSD
- Less common: OCD, illness anxiety d/o
- Factitious d/o: malingering
Management
- Drugs - SSRIs or SNRIs are first line, then TCAs, Benzodiazapines, MAOIs
- Psycho-education
- Psychotherapy (CBT)

Specific phobia
Disproportionate fear of a specific object or situation, provoked by mere anticipation or exposure to object or situation, lasts more than 6 months
5 types of specific phobias:
- Animal
- Natural environment (storms, heights, etc.)
- Blood, injection & injuries
- Specific situation
- Diverse group which includes choking, vomiting & contracting illness
Two components of condition:
- Anxious anticipation or anxiety endured during feared situation or exposure to thing
- Avoidance of the situation or thing

Agoraphobia

Leads to avoidance of places - supermarkets, churches, freeways
Avoidance, fear or anxiety causes significant impairment to routine or daily fx (work &/or social)
Duration of more than 6 months in at least 2 of these situations: public transport, open spaces, enclosed spaces, standing in line/being in crowd, being alone outside of home
