LUNGS PATHOLOGY 1
-
ACUTE LUNG INJURY
-
etiology
-
predisposing conditions
- shock
- oxygen therapy
- sepsis
-
acute exudative stage
- endothelial activation: TNF secretion by resident alveolar macrophages
- adhesion and extravasation of neutrophils: degranulation and release of inflammatory mediators
- accumulation of intra-alveolar fluid and formation of hyaline membranes: damage and necrosis of alveolar pneumocytes, leading to surfactant abnormalities; accumulation of protein-rich edema fluid and debris
- proliferative or organizing stage: type II pneumocytes proliferation, granular tissue formation in alveolar walls and spaces
- late fibrotic stage: granulation tissues evolve into scarring of the alveolar septa and spaces
