MOA
block ATP sensitive potassium channels on B cell of pancreas, increasing insulin secretion
Side effects
hypoglycemia, weight gain, beta cell burn out

DPP4 inhibitors
Linagliptin, Saxagliptin,
sitagliptin, alogliptin.

side effects: GI disturbances, autoimmune skin conditions(bullous pemphigoid), pancreatitis

GLP 1 receptor agonists
liraglutide(daily), semaglutide(weekly)
moa
Binds GLP1 receptor, enhancing the incretin effect i.e. increases glucose-dependent insulin secretion and decreases glucose dependent glucagon
side effects
gi disturbance, pancreatitis, medullary thyroid cancer, retinopathy
advantages
weight loss, reduced BP, reduced lipids, satiety, psoriasis

SGLT2 inhibitors

Inhibit SGLT2 transporter in proximal convoluted tubule - prevents reabsorption of glucose + increases excretion via kidneys
side effects: Euglycemic DKA, UTIs, genital infections, volume depletion, OH, rise in LDLs and HDLs

Good evidence for secondary prevention of CVD (EMPA-REG, CANVAS, DECLARE-TIMI 58)

Metaglinides
repaglinide

not often used clinically, helps with post prandial hyperglycemia (high blood sugar after a meal)
caution with CYP3A4, hepatotoxicity is often a concern

bind to peroxisome proliferator - activates PPAR-gamma in adipocytes - promotes adipogenesis + fatty acid uptake - reduced circulating lipids - increased insulin sensitivity
