Headache - Coggle Diagram

- - - - Prodrome: Non-specific symtpoms which can precede main migraine symptoms
      - Aura: Scintillating scotoma, Waxy lines, metamorphobia, focal neurologic phenomena, parestheisa, Cortical Spreading deepression most likely cause and occurs in occipital lobe
      - Postdrome
        
        Fatigue, lowered intellect, lowered mood, poor concentration/comprehension
  - - - Increase in metabolism may result in vasodilation which could contribute to pain
    - - Trigeminal nucleus and nerve fibers to blood vessels
      - Complex may be activated in migraine
    - - Moedulates trigeminovasular pain transmission
    - - Has direct connections to trigeminovascular neurons,a nd symapthetic/PSNS brainstem
    - - Central relay station for sensation
      - Migraine therapies such as triptans, and STMS works here as well
    - - Abrnoal sensory responses in migraineurs, increased sensitivty to different sensory stimuli in migraineurs
  - - - Avoid changes in sleep, meals, regulate stress
    - - Stress, certain foods (chocolate, aged cheese, red wine, nuts avocados)

  - - - Meidcation overuse could be response to increasing headache frequency
      - Medication use or withdrawal may be a trigger for primary headache disorder
      - Medication overuse may cause de novo secondary headache disorder
      - +ve feedback loop may be present
    - - Ergot Triptans, opioids: >10 days per month fro 3 months
      - Simple analgesics: >15 days per month for >3 months
      - Combination analgesic overuse headache: >10 days per months for >3 months
  - - - +5 attacks with the following: severe unilateral pain, one of the following (conjunctival injection, nasal congestion, eye edema, forehead sweating, miosis/ptosis, restlessness), attacks occur anywhere from 8 per day to over other day, no other disorder
    - - Diurnal rhythm: Typically occurs at same time, autonomic symptoms
  - - - 3 unilateral face pain with: OCcuring 1+ more divisions of trigeminal nerve, pain has at least 3/4 of following (recurring in paroxysmal attacks, severe intensity, electric shock-like qualities, recipitated by innocuous stimuli), no other neurological deficit or diagnosis
    - - Lancinating shock like precipitated by lightly touching affecting area
    - - Lesion in root entry zone
    - - Carbamazepine
  - - - Dull temporal headache, vision loss, jaw claudication
    - - Increase CRP, ESR, definitive by temporal artery biopsy
    - - Immediate high dose steroids, then another immunosupressive
