Acute Stress
Description:
Severe reactions to a traumatic event
lasting from 3 days to 4 weeks.
Diagnostic Criteria: Exposure to trauma, dissociation, intrusive symptoms, mood changes.
Etiology: Traumatic event, genetic predisposition.
Top Comorbidities: PTSD, anxiety disorders.
PTSD
Description: Persistent distress due to exposure to traumatic event.
Diagnostic Criteria: Exposure to trauma, intrusive symptoms, avoidance, negative changes in mood and cognition.
Etiology: Traumatic event, genetic vulnerability.
Top Comorbidities: Mood disorders, anxiety disorders.
Adolescents & Adult: over six years of age. Children: Six years and younger.
Duration: last more than 1 month.
Hyperactive Amygdala
& Anterior cingulate cortex.
Hypoactive Ventromedial prefrontal cortex.
Reduced volume of the hippocampus
Treatment:
CBT, Cognitive processing therapy, Prolonged exposure, Conditional recommendation for brief eclectic Psychotherapy, EMDR.
Treatment for children:
Trauma-focused cognitive behavior therapy ( 3-18 years of age) have sexual abuse experience, Family therapy, Parenting skills training, Conjoint Parent child therapy
SSRIs fluoxetine, paroxetine, and sertraline and SNRI venlafaxine
Adjustment
Diagnostic Criteria:
Emotional or behavioral symptoms
within 3 months of stressor onset
, disproportionate reaction.
Etiology: Stressful life events, vulnerability factors.
Top Comorbidities: Mood disorders, anxiety disorders.
Reactive Attachment
(a) a lack of seeking or responding to comfort when distressed and (b) social and emotional disturbances that include at least two of the following: minimal social and emotional responsiveness to others; limited positive affect; unexplained irritability, sadness, or fearfulness when interacting with adult caregivers.
Diagnostic Criteria:
Inhibited or emotionally withdrawn behavior, limited positive affect.
Etiology: Early neglect, disrupted attachment.
Top Comorbidities: Conduct disorders, other trauma-related disorders.
Onset:
symptoms
before age 5
, and have developmental age of at least 9 months
Disinhibited Social Engagement
Diagnostic Criteria: Indiscriminate social behavior, lack of checking with caregivers.
Etiology: Early neglect, attachment disruption.
Top Comorbidities: Conduct disorders, other trauma-related disorders.
