Red Flags
Tumour
persistent, non mechanical bone pain
pain at night or rest
unexplained weight loss
previous cancer
hard, localised mass
unexplained lump - increasing in size
sudden onset of pain - ?pathological fracture or osteonecrosis
Shoulder
urgent referral to secondary care
Trauma, pain, or weakness, sudden loss of ability to raise arm (with/without trauma) - ?cuff tear
increased shoulder mass or swelling - ?malignancy
red skin, painful joint, fever, systemically unwell - ?septic arthritis
trauma with loss of rotation and/or deformity - ?dislocation
new symptoms of inflammation in multiple joints - ?inflammatory arthritis
urgent same day referral
suspected joint infection
unreduced dislocation
acute trauma - use clinical judgement
consider urgent referral
any systemic symptoms
undiagnosed severe shoulder pain or restricted ROM
if seen acutely with history of trauma
urgent referral if suspected
malignancy
acute RC tear
inflammatory arthritis
neurological lesion - unexplained muscle wastage, loss of motor or sensation
