"This (agreement) is made on (date) by and between Party A, a (State and Type of Entity) (the "Seller") and Party B, a (State and Type of Entity) (the "Purchaser"). The parties are referred to singularly as "Party" and collectively as the "Parties".
In the case of a third party, they will be mentioned after the Seller and the Purchaser.
