Perception/cognition: Severe left neglect complicated by hemisensory loss. Attention, cognition appear WFL.

Sensation is severely impaired with two out 10 correct responses for light touch, pain, proprioception, and deep pressure throughout the left side.
Balance is poor with the need for maximum assistance x 1 to maintain upright sitting on the edge of the mat. Standing balance is not appropriate to test.

Impulsive, unable to carry over hemidressing techniques. Supported sitting. Dependent for his lower body, bed level.

Grooming: Will try to comb hair, use electric razor, and brush teeth but impulsive and not thorough.
Bathing: Maximum assistance dependent. Able to wash face after set-up; completes some grooming tasks and upper body washing, but not thoroughly. Left neglect evident.
