Impaired physical mobility
Inspect the skin regularly
Reposition the pt every 2h
Carry out active or passive ROM to all joints
Encourage pt to assist with movement and exercises
Set goals with pt for participation in activities
Medical management
CT Scan brain
MRI
Carotid ultrasound
Echocardiogram
Angiogram
Impaired verbal communications
Assess the severity of impaired verbal communications
Provide other ways of communication
Anticipate and provide the pt needs accordingly
Speak in normal tones and avoid speaking too fast
Encourage pt to continue communicating in his or her preferred mode of communication
Health education
Advise to continue with th emedications prescribed by doctor
Exercise regularly within pt tolerance levels to maintain normal BMI
Stop smoking
Recognize signs and symptoms of stroke
