transient tachypnea of the newborn (TTN)
Definition
a parenchymal lung disorder
caused by a delayed resorption and clearance of fetal alveolar fluid
characterized by
pulmonary edema
the most common cause of respiratory distress in late preterm and term infants==> The most common cause of NRD (1.3% of births)
a benign, self-limited condition
usually rapid improvement in 48 hours
Clinical findings
Immediate NRD
Favored by cesarean delivery
caused by less catecholamines secretion which promote resorption
Moderate sometimes isolated tachypnea
Absence of chest compression
Chest radiograph
increased lung volumes
flat diaphragms
mild cardiomegaly
prominent vascular markings
sunburst pattern originating at the hilum
Decreased pulmonary transparency
interlobar fissures
pleural effusions
Alveolar edema may appear as fluffy densities
Management
Supportive care only
Neutral thermal environment
Nutrition
Respiratory support
