Key triad: jaundice, pale acholic stools, and dark urine.

Etiology
Viral exposure (e.g., Reovims type 2, cytomegalovirus, rotavirus) during the perinatal period can cause biliary damage.

Ductal plate malformation theory: poorly supported ducts rupture, leading to intense fibrosis and obliteration.

Management

Kasai Procedure:

Excision of extrahepatic biliary tree, transection of fibrous portal plate, and Roux-en-Y limb
-

Liver Transplantation:
Indicated if the diagnosis is made beyond 3 months, decompensated cirrhosis is present, or Kasai fails.

Investigation
Laboratory:
Conjugated hyperbilirubinemia, increased GGT, alkaline phosphatase, and transaminases.

Triangular cord sign: A well-defined triangular area of increased echogenicity at the porta hepatis, corresponding to fibrotic ductal remnants.
