What does it look like
One end of the rod is heated, the molecules gain energy and vibrate faster.
These molecules collide with their less energetic neighbors, transferring energy to their neighboring molecules which in turn gain kinetic energy.
This transfer of energy from molecule to molecule continues until the cold end reaches the same temperature as the hot end.

Conduction of thermal energy in metals is far better than in other solids. Because other than vibrating molecules, the free electrons in a metal are also responsible for conduction of thermal energy

Convection in liquid
When the liquid is heated, it expands, become less dense and rises up to the surface.
When it reaches the surface, the liquid is cooled; it contracts, become denser and sinks to the bottom.

Radiation
Radiation does not require any medium - it can take place in a vacuum.
Radiant heat is infra-red (IR) radiation.
Radiation is the means by which energy reach us from the Sun. As there is a vacuum between the Earth and the Sun, conduction and convection cannot takes place.
