Communicating different types of documentation is a vital aspect of the therapy process. Communicating with a patients Primary care provider to make them aware of their patients plan of care and their progress from each visit. Depending on whether a therapist is in an patient or outpatient setting, the patients progress could be automatically sent to their PCP at the hospital or rehab setting or must be emailed or fax to their PCP
Keeping families informed of treatment progress is also vital since many OT patients have suffered a traumatic injury that leaves them to depend heavily on their family members. Another part of therapy for outpatient patients is involving an at home exercise program which may involve help from said family members. This piece of witting must be easily understandable to the point where there is no confusion on what exercises or tasks need to be accomplished at home
Documentation is is every part of the therapy process. Intake forms are completed before the patient is seen for their initial evaluation which involves their basic information, when their injury occurred, the effects from it, or even their last surgery date. Further documentation is taken during the duration of their therapy journey in the form of the outline of their treatments, their progression in the form of daily and long term notes. The last part of the documentation process is a [patients discharge report after they have reached the goals their therapist set for them during their initial evaluation.
