People affected by groupthink usually feel pressures to conform to the views expressed by an influential group leader (hesitate to voice concerns for fear of being ashamed or ostracized, assuming all group members approve of the decisions being made
Groupthinking most often arises in homogenous, insulated groups that posses no clear guidelines for decision making

Structural Factors
Group insulation: group ability to remain unaffected by out influences and easy to occur if group member do not solicit the opinions or views of those external to the group
Lack of impartial leadership: group members that are lead by people who have a personal interest in the outcome
Lack of decision-making procedures: failing to have previously established norms for evaluation of problems can foster groupthink and may be influenced by dominant voices; go along with those who choose to speak up
