“A Kidney Buster”
Acute Kidney Injury
-
-
-
Sign and Symptoms
-
Volume overload
leg edema, shortness of breath, facial puffiness
Uremia
nausea, vomiting, general malaise
-
-
-
-
-
shock
types
Obstructive
Caused by the blockage of blood flow in the heart or large blood vessels (Decrease in cardiac output)
Cardiogenic
Caused by the heart's inability to pump effectively due to muscle dysfunction (Decrease in cardiac output)
Distributive
Caused by a widespread dilation of small arterioles and small venules, resulting in blood pooling and decreased tissue perfusion (Decreased peripheral vascular resistance)
Anaphylactic
Allergic reaction triggers histamine release to cause blood vessel dilation as well as bronchoconstriction. Increased capillary permeability causes fluid to flow into interstitial space.
Septic
Caused by a systemic bacterial infection. Inflammatory/immune responses are activated causing vasodilation and increased microvascular permeability (causing fluid to flow into interstitial space.)

KIDNEY FUNCTION MARKERS
SODIUM Na
- extracellular electrolyte
- controls water balance
- & blood pressure
Raised in
- dehydration
POTASSIUM K
- intracellular electrolyte
- controls cellular pumps
& receptors
- via electrical potential
Red flag
- if high concentration in blood :
UREA
marker of
- acute renal dysfunction
eg distress, or dehydration - check Na levels
CREATININE
marker of
- chronic renal dysfunction
eg renal stone
eGLOMERULAR
FILTRATION
RATE
general marker of
kidney function
Used to
- diagnose chronic kidney disease stage
- confirm renal dysfunction as cause of
eg renal anaemia
