Formative
Definition: Formative assessments are a low-stakes assessment that teachers use FOR instruction, such as using an exit ticket in order to gauge if you need to reteach anything from your lesson today.
Fist to Five: A teacher asks their students to show them fist to five which means 0-5 fingers up to represent how confident they feel about what they just learned. I do this with my students in science to see if there's anyone that I could do a quick refresh with next class.
One minute essays: Give your students a prompt on the board and tell them that they have one minute to write their response using whatever knowledge they gained from your lesson that day. This lets you know what information they have off the top of their head.
Interim/benchmark
Definition: These assessments are used to see where students are during different points in the school year to determine if they are on track or falling behind. This is also a type of assessment that can be used to inform and modify your teaching based off of the results.

District Assessments: Students are given an assessment over all of the units that they learned during the nine weeks to see where the student falls after a longer period of instruction.
Weekly spelling tests: Student will be given a spelling test each week to determine if they have met the learning goal that was intended for that week.
Summative
Definition: Summative assessments are assessments that are used to SUM up the learning that has been done at the end of a unit or topic study. These assessments are usually for a grade and much more time consuming than the formative assessments that came before them.

Research projects: Students are given a specific topic or historical figure that relates to the unit that they just learned and they have to answer a series of research questions in order to present their findings. These are also typically graded from a rubric.
Midterm exams: An exam that is given to students to see what they have learned so far this semester. They are also the bane of my existence :)
Performance
Definition: A performance assessment requires students to apply critical thinking and use a deeper understanding of the content to perform and complete tasks instead of answering test questions or writing essays.

Completing a lab: Students will be given a set of lab instructions and a intended goal, then they will have to follow directions and come to a conclusion based off of their findings using the knowledge that they've gained throughout the unit.
Debates: Students will be given their debate topic beforehand so they can prepare their information, and then they will come into class and perform a debate based off of their learning and the stance that they were given.
Diagnostic
Definition: Diagnostic assessments are used as a pre-assessment to identify a student's strengths and weaknesses in an upcoming area of study, as well as determine any misconceptions that need to be targeted during instruction.

Monthly IXL assessments: Students are supposed to "Step into the Arena" at the beginning of every month to diagnose what their current levels are in each skill area in math in order to see where they are.
Surveys: Students can take a survey as a pre-assessment to see what they already know about an upcoming topic and what they do not know or have misconceptions about.
