Synthetic broad spectrum antibacterials
Core of two fused 6- membered ring, can be substituted to form fluoroquinolones (main for bacterial infections)
Inhibit DNA replication by inhibiting, DNA topoisomerase (gyrase) & topoisomerase IV (not present in humans) - so specific and bactericidal
First Gen
Used # gram (-) organisms
Uncomplicated UTI, little use (cause carcinogenic)
Second gen
Used # gram (-), Pseudomonas, and some gram (+)
Not # S. Pneumoniae
Uncomplicated & complicated UTI, pyelonephritis, STDs, prostatitis, skin/soft tissue infection
Third generation
Used # gram (-), Pseudomonas, and expanded # gram (+) including penicillin sensitive & resistant S. Pneumoniae
In Acute exacerbation of chronic bronchitis, community aquired pneumonia
Fourth gen
Used # gram (-), Pseudomonas, and expanded # gram (+) including penicillin sensitive & resistant S. Pneumoniae And broad anaerobic coverage
pyelonephritis, STDs, prostatitis, skin/soft tissue infection, Acute exacerbation of chronic bronchitis, community aquired pneumonia
, plus abdominal infection, nosocomial pneumonia, pelvic infection
Occasional adverse effects like tendon problems in all ages/ avoided in athletes
CI: myasthenia Gravis, epilepsy, QT prolongation, CNS lesions, children under 18
Orally/ injection. Well distributed in CSF, eye, prostate
