Abd. aortic aneurysm (AAA)
AAA is a potentially life-threatening condition with abnormal dilation and weakening of the abdominal aorta.
If ruptures, it can cause severe internal bleeding and requires emergency surgery.
Causes:
AAA is often associated with atherosclerosis , hypertension, smoking, and genetic factors.
Investigations:
Imaging tests such as US, CTA, or MRA.
Treatment:
Surgical repair
Endovascular aneurysm repair (EVAR)
Sometimes conservative management depending on the size, symptoms, and risk of rupture.

CP:
abdominal pain, back pain, and pulsatile abdominal mass
Acure aortic dissection
Investigations
Chest X-ray: May show widening of the mediastinum (the space between the lungs) due to the dissection.
ECG (electrocardiogram): May show abnormalities, such as ST-segment elevation or depression, indicating myocardial ischemia.
CT scan : The gold standard for diagnosing aortic dissection, as it can provide detailed images of the aorta and surrounding structures.
MRI (magnetic resonance imaging): May be used to confirm the diagnosis or to evaluate the extent of the dissection.
Angiography: May be used to evaluate the blood flow through the aorta and to guide treatment.

Emergency surgery: In cases of life-threatening complications, such as organ ischemia or cardiac tamponade, urgent surgical intervention may be necessary to repair the aorta and restore blood flow.
Medications: Blood pressure-lowering medications, such as beta-blockers and calcium channel blockers, may be used to control blood pressure and prevent further dissection.
Endovascular repair: In less severe cases, endovascular repair, which involves inserting a stent into the aorta to reinforce the weakened area, may be an option.
Close monitoring: Patients with aortic dissection require close monitoring to detect any complications and to adjust treatment as needed.
Lifestyle modifications: Patients with aortic dissection may need to make lifestyle modifications, such as quitting smoking, reducing stress, and maintaining a healthy weight, to prevent further damage to the aorta.
Causes
Hypertension is the most common cause
Atherosclerosis can weaken the aortic wall and increase the risk of dissection.
Connective tissue disorders, such as Marfan syndrome or Ehlers-Danlos syndrome, can weaken the aortic wall and increase the risk of dissection.
Trauma, such as blunt chest trauma, can cause aortic dissection.
Tear in the inner layer of the aorta, causing separation of the layers.
CP
Sudden, severe chest or back pain, often described as tearing or ripping pain.
Pain that radiates to the neck, arms, or abdomen.
Pulse deficit (a difference in pulse strength between one arm and the other).
Syncope (fainting).
Hypotension (low blood pressure).
Cardiac tamponade (accumulation of blood around the heart).
Organ ischemia (reduced blood flow to vital organs).

Renal Artery Thrombosis
Acute Superior Mesenteric Artery Syndrome: Compression of the duodenum (the first part of the small intestine) by the superior mesenteric artery, leading to obstruction.
Acute Inferior Mesenteric Artery Syndrome: Compression of the colon by the inferior mesenteric artery, causing obstruction.
Acute Celiac Artery Compression Syndrome : Compression of the celiac artery by the median arcuate ligament, causing reduced blood flow to the stomach and other organs.
Acute Splenic Infarction
Acute Hepatic Artery Thrombosis: Formation of a blood clot within the hepatic artery, leading to reduced blood flow to the liver.
Aortoentric fistula (rare)
Acute mesentric ischemia
Causes: Mesenteric ischemia can be caused by arterial embolism, arterial thrombosis, mesenteric venous thrombosis, or non-occlusive mesenteric ischemia.
Investigations: Diagnostic tests may include blood tests, abdominal imaging (CT, CTA, or MRA), and sometimes mesenteric angiography.
Treatment: Treatment depends on the underlying cause and severity of the condition.
Anticoagulation therapy to prevent further clot formation
Surgical embolectomy or
Thrombectomy,
Stenting or bypass surgery.

Splenic a. aneurysm rupture
Causes: Rupture of a spleen can occur due to trauma, atherosclerosis, pregnancy, or connective tissue disorders.
Investigations: Imaging tests such as ultrasound, CTA, or MRA can help diagnose the aneurysm and determine if it has ruptured.
Treatment: Immediate surgical intervention is required to control bleeding. This may involve splenectomy or repair of the aneurysm.

Portal vein thrombosis
CP
causing jaundice, and abdominal pain and ascites (liver damage).
Causes:
The risk factors for portal vein thrombosis include liver disease, cancer, and surgery.
Investigations:
ultrasound, CT scan, or MRI.
Treatment
Anticoagulation therapy, thrombolysis, or surgery to remove the clot or bypass the blocked vein.
