Alpha Adrenergic Agonist
Mechanism of Action
❑ Alpha agonists work by both decreasing production of fluid (reduce
aqueous humor formation) and increase drainage.

❑ Apraclonidine Ophthalmic Solution 0.5%, 1% (Iopidine®)
❑ Brimonidine Ophthalmic Solution 0.1%, 0.15% (Alphagan P®)
Cholinergic Agonist
Mechanism of Action
Pilocarpine
Acts through direct stimulation of muscuranic receptores and smooth muscle including the iris and secretory glands
-Produce miosis and contract the ciliary musle
-Causing increased tensuin on scleral spur and opening of trabecular mesh work spaces to facilitate out flow of aqeous humor
-Outflow resistance is reduced , lowering intraocular pressure(IOP)
Carbachol
Acts through muscarinic and nicotinic receptor
-It mimics the effects of Ach
-Causing miosis and a spasm of accommodation in which the ciliary muscle of the eye remains in a constant state of contraction
Side Effect
This is due to constriction of the pupil. Miotics increase drainage of intraocular fluid by making the pupil size smaller, thereby increasing the flow of intraocular fluid from the eye.

Prostaglandin Analogues
Medications:
Latanopros Opthalmic Solution0.005% (Xalatan)
Bimatoprost Ophthalmic Solution 0.03%(Lumigan)
Travoprost Ophthalmic Solution 0.004%(Travatan)
Mechanism of Action:
It reduces the intraocular pressure (IOP) by increasing the outflow of intraocular fluid (aqueous humor)from the eye.
Side Effect
Latanoprost:
Increase in brown pigmentation of the iris, Blurred vision, Burning and
stinging, Foreign body sensation, Itching, Punctate epithelial keratopathy,
Conjunctival hyperemia, Upper respiratory tract infection/cold/flu, Dry eye,
Excessive tearing, Eye pain, Photophobia.
Bimatoprost
Conjunctival hyperemia, Growth Ocular burning of eyelashes, Ocular
pruritus, Ocular dryness, Visual disturbance.
Travoprost

Blurred vision, Cataract, Conjunctivitis, Dry eye, Iris discoloration,
Foreign body sensation, Pain, Pruritus, Abnormal vision, Blepharitis,
- Ocular hyperemia, Decreased visual acuity, Ocular discomfort,
- Increased thickening, elongation & growth of eyelashes,
