Drugs for Dyslipidaemias
-
Bile Acid Sequestrants
-
-
(+) charged drug, binds onto (-) charged cholesterol (in the form of bile acids) to be excreted and preventing them from entering enterohepatic circulation and hence upregulating LDL receptors
-
-
-
-
-
Fibrates
-
Increasing LPL enzyme activity, hence increasing conversion of VLDL --> LDL, and decreasing the VLDL conc.
-
ADR
-
Muscle pathology: Myositis,myopathy, rhabdomyositis
-
-
-
Nicotinic Acid
-
Best used and only drug certified for increasing HDL
HDL is the nice fat -> good for increasing the nice fat
-
In adipose tissue it inhibits HSL enzyme which inhibits lipolysis and results in reduced transport to the liver and hence less TG synthesis in the liver.
increases LPL activity. Also Increases HDL (not by increasing synthesis) but by decreasing clearance rate of apo A-I in HDL
Adverse Effects
flushing, pruritus, vasodilation

-
PCSK9 Inhibitors
Prevents the destruction of LDLR by binding onto the PCSK9 that sometimes bind to the LDLR on hepatocytes and activates destruction of LDL + LDLR. hence increases no. of LDRs to reabsorb LDLs
-
Inconvenience
Expensive, only by SC injection
