-
-
-
extra: soft tissue swelling, redness, warmth, pain
really good response w NSAIDs, initially then no resp
signs:

SMOKING
genetic predisposition: 90% have HLA-DR4
Consequences
-
foot worsening: walking difficulties,
hand issues: decr in dexterity
-
-
PPy
white cells target synovial membrane, accumulate
erode bone around cartilage -> massive inflamm in synovial membrane, capsule
driven by: T cytokines, & anti-inflamm cytokines
therapy tries to dampen T cytokines - IL1, IL15, IL6, TNFalpha
-
dx:
-
radiology:
x-rays
isotope bone scan
- shows areas of high blood flow
- shows bone marrow & areas of inflamm - early RA
ultrasound
MRI
- quickest, immediate, see inside joints
- can use Doppler to see blood flow
-
-
acute polyarthritis
-
dx:
infection
sepsis, immunosuppressed
lyme disease
rheumatic fever - post streptococcla know Jones criteria
-
-
-
